Family Dedication Request

This form is for members of Malvern Hill who are interested in participating in our family and child dedication. The word “dedication” means to consecrate or set someone apart for the purpose of honoring God. When we have family and child dedication, we are declaring our desire to raise a child in the knowledge and understanding of the Lord.

After completing this form, you will be contacted to select the best date for your family for participate in a dedication service.

Examples of responses could include:
  • a favorite Bible verse
  • a shared passion or activity that your family enjoys
  • how long you've been a member of Malvern Hill
  • how you came to be at Malvern Hill
  • an area of the church where your family serves
  • the name of the LifeGroup you attend
  • a fun/interesting fact about your family.

Please upload a picture of your family to this form or email a picture to gchestnut@malvernhill.org. Your family picture will be included in a slide during the worship service and will be shared in the member email that is sent to MHBC members on Fridays.
